Virtual Ward is coming to Norfolk Integrated Care System with its main aim of keeping patients at home during acute illness and therefore avoiding unnecessary hospital admission. This is a technological assisted approach with remote monitoring capabilities with a focus on patients with respiratory disease, frailty and heart failure.
The Virtual Ward Hub will be the first point of contact for our referrers. Clinicians across Norfolk and Waveney will be referring to the Virtual Ward and as the Hub Allocator you will support this process by providing administrative support to the Hub and Virtual Ward Teams. Once a referral is accepted, your responsibilities will include admitting the patient onto Virtual Ward and SystmOne, allocating patients onto specific ledgers via SystmOne, supporting the capacity and demand information and sharing it with the clinical teams and assisting with all non-clinical tasks. As this is a 7 day service, we will need multiple appointees for this role.
